What is the TransLoc (Percutaneous SI joint fusion)

The CornerLoc / TransLoc procedure is a minimally invasive treatment for chronic Sacroiliac (SI) joint pain. The SI joint connects the base of the spine (sacrum) to the pelvis and allows small movements that help absorb stress during activities like walking, standing, and twisting.

Chronic pain in this joint can result from arthritis, joint instability, injury, or prior back surgeries. When conservative treatments such as physical therapy, medications, or SI joint injections do not provide lasting relief, the CornerLoc / TransLoc procedure may be recommended to stabilize and support the joint.

Goal of the Procedure

The purpose of the CornerLoc / TransLoc procedure is to:

  • Stabilize the SI joint and reduce abnormal motion that may be causing pain.
  • Provide long-term pain relief when the SI joint is identified as the primary source of discomfort.
  • Offer a minimally invasive, outpatient treatment option that reduces recovery time compared to traditional open fusion techniques.
  • Serve as a next step after diagnostic SI joint injections have confirmed the source of pain.

The Procedure Overview

  • You will be taken to a sterile procedure suite and positioned lying face down on the exam table.
  • IV sedation or anesthesia may be administered depending on your provider’s plan.
  • Using X-ray guidance, the doctor carefully places small pins and implants into the SI joint to stabilize it.
  • The implants provide immediate joint support and promote long-term bone fusion over time, which helps reduce pain.
  • The procedure typically takes approximately 60 minutes, and most patients can begin walking shortly after.
  • After completion, you will spend a brief time in the recovery area before being discharged home.

After the Procedure

Follow the instructions provided by your care team:

  • Resume light activity as tolerated; walking is encouraged.
  • Avoid heavy lifting or high-impact activity until cleared by your provider.
  • Do not soak in a bathtub or lake for 3 days. You may shower.
  • Some soreness or discomfort is normal for 7–10 days after the procedure.
  • You will receive a follow-up phone call from the clinic to check on your recovery.

Expected Results

  • Pain relief may be gradual, as the SI joint stabilizes and fusion occurs over several months.
  • Most patients notice improvement in pain and stability as healing progresses.
  • Follow-up visits will allow your provider to assess results and guide activity during recovery.

Sedation Will Be Used

  • No solid foods for 8 hours before the procedure.
  • Clear liquids (water, broth, clear juice with no pulp, tea, coffee without cream) are allowed up to 2 hours before. Carbonated beverages are not permitted.
  • No gum chewing, candy, throat lozenges, or mints within 2 hours before the procedure.
  • Your nurse will review medications and provide instructions for any you should take prior to the procedure.
  • Failure to follow these instructions may result in cancellation or rescheduling.

Potential Risks

CornerLoc / TransLoc is considered safe and minimally invasive, but potential risks may include:

  • Pain or soreness at the implant site
  • Infection at the site (rare)
  • Bleeding
  • Nerve irritation or spinal complications (extremely rare)
  • Implant-related issues (rare)
  • Ongoing or persistent pain if the SI joint is not the only source of discomfort

Insurance Requirements

Insurance companies may require:

  • Documentation of physical therapy (often a minimum of 6 weeks)
  • Appropriate imaging (X-rays, MRI, or CT scans)
  • Proof of failure of conservative therapies such as medications, several Sacroiliac injections with only short-term benefit, or rehabilitation
